Welcome to Montague Charter Academy for the Arts and Sciences Health Office.
Your child is important
Please be sure to keep your child’s health information up to date with the nurse. The nurse needs to know information like: health issues (asthma, diabetes, ADD/ADHD, lactose intolerance, allergies, etc.) medications your child is taking, and significant illnesses (chicken pox, strep throat, hands foot mouth disease, conjunctivitis, etc).
Healthy children learn better
Please do not send your child to school if they are sick or have been sick with the following symptoms for 24 hours:
-Vomiting
-Diarrhea
-Fever over 100 degrees F
-Unknown Rash
This is for your child’s comfort, as well as for the protection of the other children in school.
Medication in School
Please remember that no medication can be given at school WITHOUT written doctor’s order and parent’s consent form. Medication permission forms are available in the health office, please be sure to fill out form completely. Bring medication in its original bottle so that the nurse can verify medication given, and expiration date. (English Form) (Formulario en Español)
Physical Education
Regular physical education class participation is expected. However, if your child needs to be excused for medical reasons, a note from your child’s physician should be given to the health office. If the student is excused from physical education, this will include recess. Any student wearing ace bandage, cast, or using crutches, wheel chair, etc. shall be permitted to attend school only on written permission from a physician.
Physical/Oral Assessment
California law requires children to have a physical examination within 12 months prior to or no later than three (3) months after entering 1st grade.
An Oral Health Assessment (dental check-up) is also required by May 31st in Kindergarten or 1st grade, whichever is the child’s first year of public school.
